Licensed Clinical Social Worker (LCSW) or Licensed Marriage & Family Therapist (LMFT) – PACE Program

Salary Range: $85,000 – $90,000 annually + benefits

Requirements to Apply

  • Licensed Clinical Social Worker (LCSW) or Licensed Marriage & Family Therapist (LMFT) in the State of California
  • Valid California Driver’s License and current automobile insurance
  • Basic Life Support (BLS) certification
  • Experience with assessments, behavioral health interventions, and interdisciplinary collaboration
  • Must be comfortable conducting home visits and providing one-on-one counseling both center-based and in participant homes
  • Minimum of one (1) year of experience working with the frail or elderly population preferred
  • Bilingual in English and Spanish highly preferred

Overview

We are seeking a compassionate and mission-driven Licensed Clinical Social Worker (LCSW) or Licensed Marriage & Family Therapist (LMFT) to join our PACE (Program of All-Inclusive Care for the Elderly) team in San Marcos, California. This full-time, onsite role focuses on delivering individualized behavioral health services through a mix of center-based counseling and participant home visits.

The Behavioral Health Specialist plays a key role in assessing mental health and cognitive functioning, developing treatment plans, and supporting participants’ emotional and psychosocial well-being. Working as part of an interdisciplinary team, this clinician provides both individual and group counseling while promoting independence, dignity, and quality of life for older adults.

Responsibilities

  • Perform comprehensive psychosocial and behavioral assessments for PACE participants
  • Develop and implement individualized treatment plans based on participants’ mental health and functional needs
  • Provide one-on-one and group counseling using evidence-based interventions
  • Monitor participant progress, modify treatment plans, and coordinate ongoing care
  • Collaborate with the interdisciplinary team (IDT) to ensure integrated care planning and follow-up
  • Educate participants and staff on coping strategies, stress management, and trauma-informed care
  • Document all assessments, interventions, and care plans accurately and in a timely manner
  • Represent the PACE program at community events, outreach efforts, and professional presentations
  • Participate in quality improvement and staff education initiatives
