*This position is onsite every day at St Francis Hospital - Park. 8 am to 4:30*

$64.00 an hour

This RN Case Manager / Discharge Planning contract position requires 2 years Acute Care Discharge Planning experience. You must have STRONG two years of Discharge Planning / Care Coordination in an Acute Care Hospital Environment to be considered. Experience with an Insurance company does not qualify and will not be considered. Position requires experience in Behavorial Health.

The RN Case Manager is responsible to facilitate care along a continuum through effective resource coordination

· Transition Management promoting appropriate length of stay, readmission prevention and patient satisfaction

· Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care

· Compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy

· Education provided to physicians, patients, families and caregivers

REQUIREMENTS

• Must have BLS
• Must be actively licensed as a RN in Tennessee.
• Must have two years of working with in Case Management / Discharge Planning with an employer that was an Acute Care and have a Behavorial Health background.
• Must have Titer and immunization record up to date even though this is a remote positon
