Hospice Assistant Director of Nursing (ADON)

Location: San Jose, CA

Schedule: Full-Time

Setting: Hospice Care

About the Role

Join a compassionate, mission-driven hospice team dedicated to providing exceptional end-of-life care. We’re seeking a skilled and empathetic Assistant Director of Nursing (ADON) to help lead clinical operations, support staff, and uphold the highest quality standards for patients and families.

Responsibilities

• Support the Director of Nursing in managing day-to-day clinical operations
• Provide mentorship and guidance to field and office nursing staff
• Oversee patient assessments, care coordination, and symptom management
• Ensure compliance with hospice policies and state/federal regulations
• Participate in interdisciplinary case conferences and performance improvement
• Foster teamwork, collaboration, and clear communication across departments

Qualifications

• Active RN license (BSN preferred)
• 2+ years of hospice or home health experience (leadership preferred)
• Knowledge of hospice regulations and compliance requirements
• Excellent communication, problem-solving, and leadership skills
• Proficiency in EMR/documentation systems
