Hospice Team Coordinator (Santa Rosa, CA)

LHH is teaming up with a leading healthcare organization in Santa Rosa, CA in search of a Hospice Team Coordinator to support clinical leadership and the interdisciplinary team.

The Team Coordinator plays a vital role in ensuring seamless operations, accurate record-keeping, and effective communication among staff, patients, and families. Key Responsibilities:

  • Accurately enter and manage patient data, including admissions and updates.
  • Ensure all documentation is complete, signed, and uploaded into the electronic records system.
  • Maintain and update patient records, generate compliance reports, and add coordination notes as needed.
  • Monitor medical equipment and supply orders, and communicate trends or issues to management.
  • Facilitate timely processing of physician orders and documentation.
  • Organize and upload visit records, ensuring accurate patient histories.
  • Confirm billing and care levels match for each patient.
  • Collect and upload discharge paperwork, and notify providers as needed.
  • Coordinate with the interdisciplinary team to support holistic patient care.
  • Serve as a central point of contact for staff, patients, and families.
  • Manage incoming calls and messages, ensuring responsive communication.
  • Assist clinical leadership with meeting preparation and follow-up.
  • Prepare materials for patients in facilities and perform other duties as reasonably assigned.

Qualifications:

  • High school diploma or GED required.
  • At least 2 years of experience in medical records, medical office, or business office support required.
  • Proficiency with Microsoft Office Suite (including Outlook and Teams); experience with electronic medical records (EMR) systems such as HomeCare HomeBase preferred.
  • Strong customer service, organizational, and independent problem-solving skills.
  • Effective written and verbal communication skills.
  • Ability to maintain confidentiality and demonstrate high integrity.
  • Comfortable in a busy, fast-paced office environment.

Job Type: Contract (estimated at least 3 months)

Start Date: ASAP

Location: Fully on-site, Santa Rosa, CA

Hours: Monday–Friday, 8:00 AM–5:00 PM

Pay Rate: $27–$30/hour (depending on experience)
